I reproduced the same problem in XWiki 15.10.10. It looks like the "$document" variable is  not initialized at this line https://github.com/xwiki/xwiki-platform/blob/xwiki-platform-15.10.11/xwiki-platform-core/xwiki-platform-web/xwiki-platform-web-templates/src/main/resources/templates/notification/email/macros.vm#L254.
I couldn't identify steps to reproduce, but looks like some server config.
