I haven't seen such a request before, so without enough demand from our users I wouldn't add an option for this. Creating a custom PDF template is normally the path you would take to cover such a request. If the order in which the PDF elements are rendered prevents you from writing a custom PDF template that shows the header and the footer on the cover page and ToC then I'm fine with changing the order, provided it doesn't break other things (I don't see what, but we'd have to run all PDF integration tests to be sure). Would you be willing to create a PR with the updated order, if this is indeed a blocker for your custom PDF template?
