I've had some problems here when I forgot to add users' rights to some documents that must have restricted access. Would it make sense to add some kind of information to the footer warning about the fact that a given document is freely readable or this or that right has been granted to a given group or a number of users? Yes, redesigning the Rights UI is also in my roadmap.
Do you already have an idea in mind regarding how the rights interface should work? Some time ago JV suggested using a light background color in checkboxes to show what the current right for a given user / group is when the right isn't explicitely set (for instance a 50% transparent green background in the checkbox if the XWiki.XWikiGuest user has the edit right) -> this concept might be worth exploring further...
