I don't understand well your question. It seems to me that could be reduced to how add a comment to an attachment "event" and retrieve it together with the filename. I guess this comment must be linked to each document version, not to the attachment name as this is shared by all the releases of a document. Now, it is possible to retreive version, size, author and date, but I don't know how to add and retreive a description/comment for each of them.
