Make a javascript that checks this. There is already one that checks the document content, you can add there the title check. 2. Modify the com.xpn.xwiki.web.SaveAction to refuse documents without a title. This should be configurable, as not all users want this. You can then submit a patch, so that we can integrate it in the trunk. 3. Make a JIRA issue ( http://jira.xwiki.org/ ) requesting 2, if you don't have the time to implement it.
