For the content I'd guess you'll have to modify the xhtml2fo.xsl file in the xwiki core jar in your WEB-INF/lib directory.
Would be nice if you could write a FAQ entry about this on xwiki.org if you succeed in doing what you want to achieve :) thanks -Vincent
