Something which seems easy to implement and would provide the illusion of better performance is adding a second timer which after expiration, the page is still loaded from the cache but the cache is refreshed silently after the user loads the page. just a thought, you're the coder.
Where do you mean by "after the user loads the page" ? At the end of the view action or in some daemon thread ? Hmm maybe harder than I thought.
Does that mean that a page that is not viewed that much will stay forever in the cache because it's automatically refreshed and put again in the cache ? This would be a second timer which expired sooner than the normal timer.
Anyway i don't plan to look deeply into it anytime soon, feel free to make a detailed proposal and implementation after i get rid of it in the svn ;)
"patches welcome" ;) I only suggested it thinking it might be an idea you really liked. +1 for going ahead and ignoring my silly ideas.

Hi devs,
I would like to commit my configuration based rendering cache system before 2.4M1 for it to be well tested and improved during 2.4 dev.
You can find details about it on http://markmail.org/message/tnggdulplm73cxm5
Here is my +1
