Forms, templates

In xwiki documents are created based on a template. I have a slightly different requirement. I have a form (template) and after submitting, the resultant document should show a slightly different content than the template used. For example, if the form had a field "Name" followed by a text field, then on submit, the resultant document should say "Hello, Some name". Is this possible using xwiki? Second doubt : While creating a document, if I want to track some of the user actions, say I want to store in a database whether the current user has selected a particular checkbox. How can I do this, if it is possible. I am not getting which part of the code, I have to modify. Regards Roopesh
