Hi I know, the page cache was not the problem in this case, but since Ludovic mentioned it I wanted to share our experiences on page cache. Around spring / summer while migrating old servers to (fewer) new ones we ran into performance problems. We found the page cache to be responsible and after fiddling around a while we figured out that (for us) the performance was good when the page cache has a size of around 1/3 - 1/2 of the total number of pages over all wikis on a server. e.g. on one server we have 75k wiki pages and currently a page cache of 30k and are thinking about changing it to 40k (this runs fine on 4GB memory).
