Mine problem was with headers in fact.
Update: After switching to 3.2 (even clean 3.3 M2 refused to launch) PDF

generated despite several similar headers present in included pages.
But only first TOC brings me to correct sections, others doesn't work in
the page itself and navigate to the same sections as first one in generated

Looks like I've run into a zombie bug: having two similar headings on
pages
included into one page gives exception when exporting to PDF.
I've checked JIRA and there it is marked fixed in 1.4:

Can anyone confirm a zombie bug or hint me what I might be doing wrong?
What version are you using?

should have fixed the problem for good. XWIKI-2124 only fixed it when
headers are involved.
