Thanks for the reply, I have managed to fix this problem. Even though I set
up a new wiki (with no previous user attachments) the missing icons were
part of extensions that I had uploaded prior to moving my attachment storage
to the file system. To fix this problem I ran the following script located
at
http://extensions.xwiki.org/xwiki/bin/view/Extension/Filesystem+Attachment+…
http://extensions.xwiki.org/xwiki/bin/view/Extension/Filesystem+Attachment+…
This will transfer any DB stored attachments to your file system store.
