Indeed, this sounds like a good option for making the 'add' button

nop, no need, a meta + shortcut indeed is a good choice (and technically
easier
to achieve, also by using the actual selection rather than the clicks, as
Sergiu
was directing, a few mails earlier).
The only thing is that it would be an extra click / extra button push (just
as
3/ requires an extra click), but I think that's ok.

I'm +1 for 3) since it increases the discoverability of the annotations
features. I think it's important that people can easily discover that text
can be annotated. The other options seem too complex to me and will probably
not get used by the majority of users, especially those involving only
keypresses.
