My approach is the following :
In the IV step :
When you click on the submit button your form will be submitted to a wiki
page that call the JIRA REST service and create the JIRA issue.
You can do that in two different ways :
1) Using the default way :
Set the ation attribute of the html form to the URL of the wiki page that
containing the call to the service.
you can get the Wiki page URL by using the xwiki api.
For example if you create this page : YourAppCode.JIRARestCall, that will
contains the call of the JIRA Rest service, you can get the URL of this
page in velocity using the $xwiki api
#set($pageURL = $xwiki.getURL("YourAppCode.JIRARestCall"))
<form action="$pageURL" ...>
...
</form>
Then when you click on the submit button the form will be submitted to the
wiki page (action page) and excute the code of the jira issue creation.
You can make a redirection after the issue is created or an other action.
2) Using javascript and ajax
In this case, on the click of the submit button you will send
an asynchronous request to the wiki page URL. as explained above.

The code used to call the Jira REST web service is a server side code then
you can't excute the code in javascript.
You will need a wiki page, see here how to create a new page (
http://enterprise.xwiki.org/xwiki/bin/view/GettingStarted/CreatingAPage)
Put the groovy code in that page as explained in the service documentation.
Then you can communicate with the service with javascript using Ajax
requests.

You can call the groovy code in a wiki page like a velocity code.
For exemple you can call the groovy code in the action page of your form.
